Terms and Conditions

By checking the box, I acknowledge that I have voluntarily chosen to participate in a program of progressive physical exercise, which can enhance musculoskeletal and cardio-respiratory systems. By checking the box, I acknowledge being informed of the possible strenuous nature of the program and the potential for unusual, but possible, physiological results including, but not limited to, abnormal blood pressure, fainting, heart attack or death. By checking the box, I assume all risk for my health and well-being and hold harmless of any responsibility of the instructor, facility or any other persons involved with this program and testing procedures. I understand that questions about exercise procedures and recommendations are encouraged and welcomed.

I hereby grant permission to the Golf Performance Center (TGPC) and /or The Athlete’s Performance zone (TAPZ) and /or Junior Golf Hub (JGH) to use either my or my child’s image, videotape, audiotape or interview and name in any educational publication or advertising/marketing materials (printed or electronic). This consent also serves to waive all rights of privacy or compensation.

By checking the box, I acknowledge that there are risks inherent in participating in golf programs at The Golf Performance Center (TGPC). The Member agrees to release The Golf Performance Center (TGPC) and /or Junior Golf Hub (JGH) and its personnel from any liability from personal injury property damage or loss sustained by the Member, including injury resulting in death and any financial loss or damage, directly or indirectly resulting from the Member’s activities or participation in events at The Center. The Member further waives, as against TGPC and/or JGH, all claims recourses and rights of action that the Member may have against TGPC and/or JGH as a result of such personal injury, property damage or loss.
